How do we start?

Please complete the form on the main page or email directly at magda@drbuczek.com to check availability and set up a complimentary 15-minute zoom call to see if Dr. Buczek might be a good fit for you or your child.

Insurance?

Dr. Buczek is an out-of-network provider and does not accept insurance. Please check with your insurance if you have behavioral health included in your plan. Dr. Buczek provides a monthly superbill to submit to your insurance company for reimbursement.

Costs?

Please contact Dr. Buczek to obtain information regarding fees for individual therapy.

How do I know which therapy is for me?

A thorough intake assessment will help determine treatment recommendations. Dr. Buczek takes a compassionate and collaborative approach to helping you identify and achieve your emotional and behavioral goals.

In-person or Telehealth?

Limited availability for in-person sessions on Tuesdays or Thursdays. Dr. Buczek’s office is located at 185 Madison Ave (corner of 34th St., close to the 6 train and Herald Square, as well as within walking distance from Penn Station or Grand Central Station.)

Post-intake, Dr. Buczek will collaborate on treatment planning and will determine if you may be a good fit for Telehealth.

No Surprises Act

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost.

Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.

  • Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.

  • If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

  • Make sure to save a copy or picture of your Good Faith Estimate.
